Mail the application and check made out to Going Home Greyhounds, Inc. for $40** (Do not send cash please) to:

Leased Vehicles – You will have to attach a form MV-1L filled out by you and your vehicle’s owner(dealership you lease from). This form is small and your dealership should be familiar with it. Have them complete their section and hand it or mail it back to you. Do not have them fax it back to you completed: the state must have the original signature, but again, your dealership will probably be aware of this.

You can get the fMV-1L form by going online to: http://www.dot.state.pa.us/Public/DVSPubsForms/BMV/BMV%20Forms/mv-1l.pdf or just google search: “PA DMV MV-1l”. Once the form is completed, attach it to the special plate application and follow the above instructions.

Please do not send the application or check directly to the Commonwealth of PA or PA DMV. The application must be signed and submitted by the license plate coordinator. **Fee includes state issuance fee PA Dept. of Corrections fee for plate production, and small donation to GHG.
